What shall I take to the assessment?
The following information can be helpful:
1. medications or a list of medications
2. a current job description
3. any additional medical information that you think might be relevant e.g. medical reports, x-ray reports or blood test results
(sometimes important new information becomes available after the referrer has obtained a copy of relevant medical records)
4. a support person if you wish, e.g. spouse, friend or colleague
5. curriculum vitae
6. for women, its best to wear clothing that can readily allow for a physical examination e.g. singlet underneath to allow
for a shoulder and/or back examination, loose fitting pants that can be pulled up to allow for a knee or ankle
examination. Alternatively a gown is available.
